It depends how the promotion relegation system is altered I guess. In Italy they used to have the top four promoted from Serie B and the bottom 4 relegated from Serie B. Based on final positions since 1992/1993 (i know these would have altered according to various promotions in reality), sides such as Tranmere, Stoke, Millwall (twice) and Preston would have tasted the top flight. The flip side of course is that survival would be more difficult.
