The survey has not been set up incorrectly. The third question is a conditional dependent upon the first two answers and should only be relevant to those people answer ‘yes’ to both questions. But if you answer ‘No’ to both questions the survey should use skip logic so that the third question becomes void for those people who answer ‘no’ to the first two questions. But instead, it demands that the ‘field is completed’. Consequently, the survey is meaningless.
