I can't help but think that whoever thought up the questions thinks that Slyvanian has something to do with France and that Fine French Writer is Pennsylvania. However, Slyvanian means "forestlike".

Could 20. I'll fetch the gravel be Oregon (ore gone)?

Pretty sure 10 is Kentucky. (Kentucky Derby).

I'd guess T/A and D/A are North and South Dakota, but not sure of the reason! T=top=North, D=down=South?

32. Hbneswlug

40. Jack eats a pavlova

43.T/A

44.D/A

States:

Assuming T/A and D/A are north & south Dakota but not sure why or which is which. Really not sure on the others.

UPDATE

Unsolved as yet...

28.  Switch off saw
32.  Hbneswlug
40.  Jack eats a pavlova
43.  T/A
44.  D/A
45.  Fine french writer

Remaining states...

Indiana
North Dakota
Oklahoma
Oregon
Pennsylvania
South Dakota

Thinking 40 is probably Indiana as in Anna Pavlova, though can't work out how the Indi works, Oklahoma has been said as Fine French Writer and I supect it's right, but I'm hoping not cos it's an awful clue, and I suspect Xen is right about North and South Dakota, though like him, no idea why other than both ending the same. Which then leaves...

28.  Switch off saw
32.  Hbneswlug

Oregon
Pennsylvania... just got it HB pencil/ NESW vane/ Lug ear
Pennsylvania

I think it's

o.k. (fine)

la (French)

Homa (ie Homer - who they think is a writer rather than possibly an amalgamation of poets who orally transmitted ancient Greek poems)

ok-la-Homer
