I have designed a survey which should take no longer than 1 minute to complete, and would be truly grateful if some of you could help me out and complete it for me. Questions 7 and 9 should probably have a yes/no option followed by another question saying "if yes, which" etc. Guessing you're handing this out on match days, then it makes more sense. Done though, good luck.

Seems like he's being a bit lazy and if I can see it, his tutor will too. He can thank me when my comments earn him some extra marks.

Otherwise I was trying to keep it as short as possible so people wouldn't be put off. Thanks for taking part! Thanks to everyone who has done it so far!
