The stab wound which killed him was to the subclavian vein which bled into the thoracic cavity. This torrential bleeding was not visible externally although there would have been some bleeding from the soft tissues and the other wounds but in the dark it’s not surprising the first officers on the scene missed this.

On a similar theme. Vanity sinks - never big enough. Hand dryers - never run for long enough but even if they do spray an aerosol of bacteria laden shit into the atmosphere and the punters never completely dry their hands such that water drips everywhere. Give me paper towels every time.

I use a soft case. Put the rain cover on the top of the bag and then fill the spaces with crunched up newspapers. Never had a problem.
