What I can make out and from what people are saying on here is the form has your season ticket/membership/supporter number on it. Going forward potentially your season ticket or the ticket you bought with that membership number could be blocked, denying you access into the stadium. I think that's right anyway and filling out the form takes 2 minutes so not worth the hassle of not doing it.

I believe the declaration is simply because the terms and conditions of entry have changed since you originally purchased your season ticket and you are giving them something in writing that states that you accept these new conditions.

I don't get the form. I haven't actually looked at it yet lol but why fill in a form if you've got a Covid pass anyway? If I get asked for my proof of vaccination then I'll show it, what's the form do?

I assumed it was an addition to the Terms & Conditions to enable them to ban you from the Stadium if you don't have the pass or been tested.

Beaten to it by @SexyGammonFox who I agree with more or less.

So is this form just kept in the clubs records? I have just completed it for myself & 2 friends & it is showing in my ticket history.

Do I have to print anything out?

Sorry to ask but I'm not that great on all this technological stuff

I’ve completed the form for two people on my account. I am assume that is that, and I won’t be showing the email confirmation at every game. I will have my nhs covid pass ready at the turnstile every game

Thanks very much for that. About 5 minutes after I did it, I got an email from the club reminding me to do it, as have 1 of my friends that I registered!  Obviously then you have had a positive confirmation, hopefully I will get one later.

I’ve brought 3 tickets from general sale for mates of mine and every ticket has my name on it , how can my mates who are not members or season ticket holders  , hence general sale fill in the declaration form ??

This is the same predicament as a hypothetical situation I have just thought up. Giving your season ticket to a mate for a game here and there when you can’t go is very common - so on entry do they ensure that the name on the ticket corresponds to the name on the Covid pass?

My one experience of this is that all they do is glance at your Covid pass. I don’t think they check names correspond or even that season tickets have a completed self declaration. Am I correct?

They are picking it up from the supporter number on your season ticket / day ticket

Basically it may come to the point where if you haven't accepted the disclaimer through your account on lcfc.com, your supporter number may be blocked from entering the ground

The government have made them do this as to still be allowed fans in the ground

I was also wondering that as I've bought a ticket for my cousin using my season ticket priority so it has my name on it.  I think they must be covered by this line in the declaration "I confirm that I, and where applicable, all other adults (aged 18 or over) I am attending with, will either..." even though they may be situated in another part of the ground.  I take it this obliges us to make sure they're aware of and abide by the rules.  I'll make sure he gets in ok before I enter.  There must be a fair few others in the same situation.

My brother and I are having trouble with this. We are both season ticket holders and linked as friends and family for the purpose of buying tickets. I did my declaration this morning but had no option to do his for him as a linked person.
My brother is trying to sign the declaration right now but his supporter number is not recognised.
I just went back on to double check if I could do it for him and it says all tickets under my name have been allocated.
Anyone else struggling?

I am not able to do that in the way that I have done successfully lots of times before. It’s just not an option.

I found a work around though. I had to fill out the form when I wasn’t logged in and found my brother’s membership via his postcode. Then you have to sign in. Bizarrely ‘no account found’ using his log in details, but it was when I signed in under my name - just in case this helps anyone else.

Went in just after 7:30 yesterday, they took a quick glance at the pass I was showing on my phone - no check as to whether it was valid or anything

My grandad and sister who I was with just walked straight past without being checked

No different to before the new "guidelines" were brought in

Yeah no different really.

So the implementation of the Plan B checks didn't actually get implemented?
