We constantly make so many bizarre errors in games

That right there is everything frustrating with this team. Lookman could have carried that ball and committed the defender before passing and yet he gave a slow ball to Nacho who just slowed everything down and gave Napoli the chance to regroup and stifle the attack. We should have absolutely belted down the field then, but it's always this "caution first" approach these days.
