Because players aren’t showing for the ball in tight areas and others aren’t brave enough to make the risky pass when they do ..... confidence .... effort ......

It doesn't explain everything but we don't seem to be having a great deal of luck. Kelechi was nearly decapitated against Man City with no penalty and a pretty dubious decision denied him a goal last night. But luck does turn in the end.

When we get it down the flanks we aren't a) whipping the ball in and b) getting our midfield players in quick enough. Our midfield aren't showing any urgency to get the ball and pass it at all, massive concern. I'd put Praet in as cam with Maddison and Youri in front and deeper and getting it out wide quicker which will allow the full backs more time to get forward too like they were doing in the 1st half of the season.
